CCCS Bankruptcy Counseling Initial Bankruptcy Counseling Packet (download | complete online)
Clients must complete this packet before their first Bankruptcy credit counseling appointment; that is, the appointment required for all clients seeking the credit counseling certificate one needs to file for Bankruptcy. This can be done in one of two ways:

1) Clients can either download and print a copy of this packet, fill it out, and either bring the completed packet with them to their appointment or mail to it to CCCS in advance of the appointment, OR

2) Clients can complete the packet online.

Clients must print and complete this registration form to sign up for the debtor education class clients take after they file for Bankruptcy. Please print and fill out this packet, and either mail it or bring it to the CCCS offices after filing for Bankruptcy. All required payments (see form for details) must be rendered in the form of a check or money order. Please note, a Bankruptcy case number is required to complete this registration form.
