Pre-filing bankruptcy counseling is provided in-person, by telephone, or via the Internet.

Pre-Filing Bankruptcy Counseling Application (download | complete online)
A Bankruptcy Counseling Certificate is required for all clients seeking to file bankruptcy and can be obtained in one of two ways:

1. Client can either complete the Pre-filing Bankruptcy Counseling online OR

2. Client can download and print a copy of this packet, call CCCS to schedule an appointment and either bring the completed packet with them to their scheduled appointment or mail it to CCCS in advance of their scheduled telephone appointment.

Debtor Education Class Registration Form (download / complete online)
Clients must complete this registration form to sign up for the debtor education class clients take after they file for Bankruptcy. All required payments (see form for details) must be rendered in the form of check, money order, or paid online with a VISA or MasterCard debit card or personal checking account. Please note, a Bankruptcy case number is required to complete this registration form. Clients may register for the class in one of two ways:

1. Clients can either download and print a copy of the registration form, fill it out and either mail it or bring it to the CCCS offices with the required fee (money or check), OR

2. Clients can register and pay for the Debtors Education Class online.
